* Azure: add PostgresSQL support and refactor to use resource graph
- Refactored to use @azure/arm-resourcegraph for all queries
- Refactored database lookup to do just 2 queries
(all servers, all DBS) instead of waiting serially on 1 query per RG
- Added Azure Database for PostgresSQL Servers support in the tree
- Removed use of older azure APIs in preference to ones compatible with resource graph
- Note: Had to use v1.0 of new subscriptions package because resourcegraph is 2month out of date vs all other packages
- Add SQL Instances folder and support using existing SQLClient API
- Sort subscriptions in tree and quickpick for easier search
- Sort all resources (Databases, Servers, Instances) alphabetically too
Not in this PR:
- Will experiment with Graph API for faster perf & easier addition of other Azure resources such as PostgreSQL
Consolidated most logic into a base class and common resource request pattern.
Reduces cost to add new providers, which will help for SQL Managed Instance support

- Improvement (but not full fix) for #4732 where Azure startup is very slow.
This speeds the startup by up to 5 seconds by
- Changing from fixed 5 second initial wait time to a promise to check for accounts
- Using the accounts changed notification to send a message when accounts are ready. This is usually what will "win out" since Azure seems to load before the Account Providers are set up.
- Remove right-click actions from the command palette.
- Rename Azure actions so it's clear what they are in the command palette.

VSCode serialization changed rejection handling to only serialize errors, which caused things to break
- Changed to return either account info or a cancel message in the resolve
- Rewrote to use promises. Tracking how to return canceled through 4+ thenables was way trickier than just using a promise
- Updated unit tests to handle new scenario
- Tested integration tests, realized they a) didn't run and b) didn't passed.
- Added vscode dev dependency to fix run issue
- Fixed tests to account for behavior changes in tree state.

- Moving this code from the SQL Server 2019 extension to Azure Data Studio core
- Ported tests and verified they work in the integration tests.sh file
- Fixed an issue that caused integration tests to fail if you have a SQL Server 2019 big data cluster endpoint listed, but the extension isn't installed.
